Abstract
Objective: To evaluare the prevalence of angular cheilitis associated with HIV infection, its relatio11ship with Candida infection and the main clinical and therapeuticfeatures of the disease in a Spanish popu/ation. Study design: Oral and physica/ examinations were performed on 112 HIV-infected Spanish patients. Oral swabs were samp/ ed in Sabouraud glucose agar piafes. /so/ates were identified by conventional mycological methods. Results: Angu/ar cheilitis was observed in 16 patients ( 14.4%). The risk groups were: intravenous drug users (81.2%), homosexuals ( 12.5%), and heterosexuals (6.2%). Angular cheilitis presented as a bilateral and chronic (> 1 month evolution) /esion and it was associated with e1ythematous candidiasis in seven patients ( 43. 7% ), and with pseudomembranous candidiasis in three cases (18.7%). In six persons (37.5%), angular chei/itis was the unique Candida-associated /esion. Cultures yielded yeasts in ali the clinica/ specimens from these patients: Candida albicans (78.9%), Candida tropical is ( 10.5%), Candida famata (5.3%), and Rhodotorula rubra (5.3%). Treatment with topiccil nystatin alone or in combination with oralf/uconazo/e yie/ded the best results. Recurrence was observed in seven cases (43.7%). Conclusion: Angular cheilitis is a common Candida-associated oral /esion in Spanish HIV-infected patients. The disease manifests as a bilateral, chronic, and recurren/ lesion.
