Abstract

Because the morbility and mortality rates of intracerebral hemorrhage (ICH) are high and show an uncertain initial prognosis, its clinical management in the acute phase has to be aggressive. The treatment is mainly focused to prevent hematoma expansion and intracranial hypertension (IIH). In order to prevent severe complications, all emergency treatments should be carried out at neurologic/neurosurgical intensive care unit. The adequate management of blood pressure, the rapid reversal of anticoagulation and prevention and treatment of ICH are essential. Large cerebellar or life-threatening cerebellar hematomas should be evacuated urgently. Nevertheless, because surgical indication does not improve functional outcome, its use controversial and has to be tailored in supratentorial ICH. Minimally invasive surgery shows promising results and it might indicate the path to follow in the future.
