Résumé

Introduction: To assess granulocyte apheresis (GCAP) as maintenance treatment in patients with Ocular Behçet’s Disease (BD) resistant to immunosuppressive therapy (prednisone, cyclosporin, azathyoprine or mycophenolate mofetil), in those cases where the induction tretament was satisfactory. Methods: Prospective observational study of three cases, carried out in a university centre. The intervention procedure consisted of an extracorporeal GCAP using a column filled with cellulose acetate beads (Adacolumn, JIMRO, Takasaki, Japan). After induction treatment the patients underwent a schedule of therapy of a session per month for four years. Visual acuity (logMAR), degree of intraocular inflammation and doses of immunosuppressive therapy were measured and observed every month. Results: Treatment with prednisone was reduced in all three cases. The Number of relapses reduced in two cases and were avoided in one. Visual acuity was maintained in two cases and improved in one. There were neither side effects nor adverse reactions. Conclusions: GCAP has shown to be a safe and effective maintenance treatment in cases of ocular Behçet’s disease refractory to medical treatment. It has permitted the avoidance of relapses with a reduction of the average dose of cortisone.
