Abstract

Thyroid problems have a high incidence, and many of the diseases require surgery. One of the most frequent complications is postoperative hypocalcemia secondary to postsurgical hypoparathyroidism, generally due to vascular involvement of the parathyroid glands. Biochemical hypocalcemia is observed in up to 83% of patients after surgery; however, symptomatic hypocalcemia occurs less frequently. It occurs in the first week after surgery. Despite the fact that electrolyte disturbances are often the cause of a variety of medical emergencies, hypocalcemia has a low incidence. In severe hypocalcemia, signs of Trousseau and Chvostek may be absent, which could lead to a delay in diagnosis. Hypocalcemic stridor is a vital emergency that can compromise the patient's life. It has a quick resolution if it is treated with calcium gluconate; and in case of refractoriness, an associated hypomagnesemia must be considered.
